Excludenodes not considered?

Okay.  Thanks to help from "lark" on IRC, I think we've chased this
one down.  It should be fixed in r18575 in trunk.  If anybody else can
build from source and try it out, that'd be great.

(The problem was that we had an internal data structure to represent
the union of ExcludeNodes and ExcludeExitNodes, but we weren't ever
updating it with country info from the geoip file.)
