[community] Research ethics board

I'm considering creating formal ethics forms for conducting research on,
or about, Tor for subsequent submission to The Tor Project's research
ethics board.

I feel it'll help researchers outline and explain their proposals for
conducting research in addition to formalizing it in a consistent
manner. I don't feel it'll be too difficult to do it. I intend to upload
draft ethics forms to GitHub and I'd be grateful for suggestions or
ideas or collaboration.
