Comment:

 It's hard to debug this issue and I can't think of a useful log_warn to
 add when the bug triggers so that we can detect this better in the future.
 Info logs would help us figure it out, but we might want to look deeper to
 improve logging.

 I have assigned this to Sponsor27 and I'm delaying it to 041 since this
 does not seem frequent enough or security-related.
