Comment:

 I am taking this out of 0.4.1 because none of the above cases are
 catastrophic, and fixing them with a sequence number fix will still have
 cell count issues that cause these circuits to be uniquely recognizable
 (due to the extra padding negotiation *and* cannibalization onionskin
 cells each time there is a purpose change due to a NACK). We need both the
 sequence number fix and padding machine changes to make these cannibalized
 NACKed intro circuits not stand out. :/
