 Ocx0000022 is the error "The application was unable to start correctly"

 Uninstalled, re-installed as Admin; went in to security properties of the
 Firefox exe file and verified that permissions were there for all and went
 to Windows Firewall and put in an exception for the browser.  Went and
 disabled my  antivirus and it still would not launch.  What have I missed?
