 perhaps you tested with security level set to low?

 Okay, I've tested with a fresh Tor Browser 7.5a1 and I  got our external-
 app-helper warning dialog and allowing the download of the file started it
 (verified on `about:downloads`). This worked on any slider setting. So,
 not sure what is going on in your case. Do you get the external-app-helper
 warning dialog as well and the download still fails to start? And could
 you check `about:support` if Multiprocess Windows are enabled?
