Comment (by dcf):

 I had this ("mismatch in server hash") reported before for meek-server. It
 had something to do with the file extended_orport_auth_cookie; it was
 stale or something.

 Do you have a file called extended_orport_auth_cookie under /var/lib/tor?
 What happens if you delete it and restart? Did you previously have tor
 installed, and then you reinstalled it with obfs4proxy?
