Comment:

 circuit_unlink_all_from_channel makes me think the circuitmux code could
 be involved in some stage of this.  Still not sure.  I made some verbose
 log code to try to track mismatched destroy pending/destroy un-pending
 cases. I found in the process that one case I thought was  impossible is
 actually possible, but it does not appear to be responsible.

 I don't know at this point whether this is one bug or two.
